Facelifts are very popular because almost everybody looks better after having one. However, there are many variations of this procedure and different goals. The face can be divided up into lower, middle, and the upper, which is the forehead. Specific procedures exist for improvement of each of these areas. Facial rejuvenation usually requires tightening and repositioning of the deeper soft tissue structures of the face as well as the skin. When there are fine lines in the skin as seen with some patient, some resurfacing is necessary. Note that the color and texture of the skin are improved if this is done.

Most patients first become aware of facial aging when it affects the neck. As gravity pulls on the soft tissue of the face, stretch and sag cause accumulation in the central neck. This tissue is repositioned back to its higher position during a facelift. Simpler, more direct excisions of excess tissue in the neck are sometimes done for patients with limited expectations. Some patients, more often men, accumulate large amounts of fatty tissue in the neck as they age. They often have no interest in having a complete facelift, but only want the neck improved. This tissue can be removed either directly or with liposuction. Tightening the skin is then usually done with a standard facelift where incisions are hidden around the ear. The patient below presents a typical situation.

The classic looseness in the jowl region that is usually a primary motivation for seeking facial rejuvenation. The telltale signs of a facelift are often found around the ear where the incisions are made. Our patient's scars are very difficult to find even on close inspection. An otherwise good facelift is ruined if the ears appear distorted and scarred. Problems around the ear can be difficult to correct, so it is best to see examples of your surgeon's work before you commit to surgery.
